Registration for the 2012 - 2013 season is open!
Medical Information and Consent to Treat Forms
Please give these to your manager or coach at the first practice.

Coach and Volunteer Registration Information
All coaches and volunteers must register with USA Hockey. Then complete the background check at the Minnesota Hockey Website (if 18 or older). Then email your USA Hockey Registration confirmation number to Colleen Simcoe at colleen@mnspecialhockey.org
If you are volunteer that is going to be on the ice, you must sign the Waiver of Liability before you can be on the ice. Please give this form to your team manager or head coach at your first practice.

Please follow the instructions on this page. Enter "Eagan Hockey Association" for Hockey Association. Enter "No Associated District" for MN District.
